Alex is fifteen years old and lives with their parents, Kraken and Suli, in a wooden cabin by the sea, far from Buenos Aires. Alex is intersex and until now has been raised as a girl, taking medication that suppresses the masculine traits of their body. Their parents chose this isolation so the teenager could grow up free of prejudice, protected and at peace. But Alex decides to stop taking the medication without telling anyone, and their body begins to change. The family is thus faced with a situation they were not prepared for, while the protagonist tries to figure out what identity they want to build. The film is directed by Lucía Puenzo and stars Inés Efron and Ricardo Darín.

The title refers to Klinefelter syndrome, also known as XXY syndrome, but the Italian organization Unitask considers it confusing, because men with that syndrome do not have feminine traits.

Inés Efron won the Best Actress award at the Cartagena Film Festival, along with three other awards.

According to the review aggregator Rotten Tomatoes, 82% of critics gave the film a positive review, based on 44 reviews.

The film was selected by Argentina to compete for the Oscar for Best International Feature Film in 2007, though it ultimately did not receive a nomination.

In her feature film debut as a director, Lucía Puenzo received support from Cinéfondation, Fond Sud, INCAA and ICAA to produce the film.

The film won nine Sur Awards from the Argentine Academy, including Best Director and Best Actor for Alex Brendemühl.

In an interview about the film's distribution in Argentina, Puenzo stated that XXY opened with 20 prints, compared with more than 200 for Spider-Man 3, and pointed to the difficulties of staying in theaters.
