After graduating, he moved to New York and began studying at Neil Simon's Biloxi Blues. In 1985, he was cast as Woody Boyd, the naive and friendly bartender on the TV series Cheers, which earned him an Emmy, among other awards. His first film role was in Wild Cats in 1986, alongside Goldie Hawn. He continued taking small film parts until the romantic clash he had with Michael J. Fox in 1991's Doc Hollywood.

Harrelson later worked on films such as The Thief Who Came to Dinner, Wag the Dog, White Men Can't Jump, City Slickers, Indecent Proposal, The Hi-Lo Country and Oliver Stone's Natural Born Killers (1994), in which he plays a ruthless killer.

In 1996, he starred in the controversial film The People vs. Larry Flynt, which tells the story of businessman Larry Flynt, and earned him an Oscar nomination for Best Actor. In 2001, he returned to television for seven episodes of the NBC comedy Will & Grace. He played a transgender prostitute in the film Mr. Right, starring Adam Sandler and Jack Nicholson, and FBI agent Stan Lloyd in The Hateful Eight.

In 2006, he appeared in The Last Show and Ain't Nobody's Business. The drama The Messenger premiered at the Sundance Film Festival in 2009, winning the Silver Bear for Best Screenplay at the Berlin Film Festival. After a decade in the shadows, he regained fame with No Country for Old Men, the Coen brothers' film that won four major Academy Awards in 2008. In 2009, he played Charlie Frost, a radio host, in the film 2012. That same year, he starred in Zombieland as Tallahassee, a zombie hunter. In 2012, he played Haymitch Abernathy, the drunken mentor of the protagonists in The Hunger Games; he played the same character in the next three films of the franchise.
