Craig Gilner, a 16-year-old teenager struggling with depression, goes to a Brooklyn hospital because he is considering suicide and needs help as soon as possible. A doctor admits him for five days, but the adolescent psychiatric ward is closed, and he has no choice but to stay in the adult unit. There he lives alongside patients much older than him, each with their own problems, and Craig has to adapt to an environment he never expected. Bobby, another patient, becomes his mentor and protector. The film is directed by Anna Boden and Ryan Fleck, and the lead role goes to Keir Gilchrist, joined by Zach Galifianakis.

Principal photography lasted about six weeks, from November 30, 2009 to February 2, 2010, and took place in New York.

The movie's hospital was not shot on a studio set but at a real medical center in Brooklyn that served as Argenon Hospital.

The Canadian indie rock band Broken Social Scene worked on the film's score.

Hyperion Books released a new edition of the novel to coincide with the film's release, featuring movie stills on the cover.

Focus Features went from a limited release to a wide release in about 500 theaters across the United States.

The film grossed 6,363,628 dollars in the United States and Canada, 269,322 in other markets and 6,632,950 worldwide, against an estimated budget of eight million dollars.

Zach Galifianakis agreed to play Bobby after Anna Boden and Ryan Fleck called him to discuss the project and the tone they were aiming for in the film.

During filming, Galifianakis worked in an abandoned wing of a hospital that was still operating, a circumstance that helped create the atmosphere of the psychiatric center shown in the movie.
