John Trent, a former insurance investigator, arrives at a psychiatric hospital by ambulance, violently agitated and strapped down. They lock him in a padded room, where he suffers disturbing hallucinations that make him doubt what he sees around him. Shortly after, Dr. Wrenn sits down across from him, and Trent begins to tell him his story. The account takes him back to the days before, when he accepted an assignment that pulled him away from his routine: tracking down a missing horror novelist. Directed by John Carpenter, the film centers on Sam Neill, who plays Trent, and the tension between what he remembers and what those around him perceive.

Trivia about In the Mouth of Madness

Michael De Luca wrote the screenplay in the late eighties, and one of the first directors he offered it to was John Carpenter, who initially turned it down.
The main theme, heard during the opening credits, was inspired by Metallica's song "Enter Sandman." Carpenter wanted to use it, but he couldn't secure the rights, so he composed his own.
KNB EFX Group took seven weeks to create the practical effects, including an 18-foot wall of monsters mounted on rollers and operated by twenty-five people.
The novelization of 2025 is credited to the pseudonym Sutter Cane, although it was written by Christian Francis, editor in chief of Echo On, the imprint of Arcane Publishing that published it.
